ACA Open Enrollment 2027 in San Antonio: All 9 Insurance Companies in Bexar County & Their 161 Plans

Open enrollment for 2027 health coverage opens tomorrow — Sunday, November 1 — and Bexar County shoppers get more choice than they have had in years: 161 plans from 9 insurance companies. The catch is price. The extra federal help that held premiums down from 2021 through 2025 expired at the end of last year, and Congress has not brought it back. This page lists every company selling Marketplace plans in Bexar County, what changed for 2027, and the dates that decide when your coverage starts.

These are the companies and plan counts HealthCare.gov’s plan preview shows for a San Antonio ZIP code (78207), checked October 30, 2026. Your final price depends on your age, income, and household — the preview shows the plans; enrollment opens November 1.

One local note: Community First Health Plans is San Antonio’s own nonprofit plan, run by University Health — if your doctors are in the University Health system, its plans are worth a close look. Blue Cross has the widest doctor network in Texas. Every Bexar plan is an HMO or EPO style plan, which means checking that your doctors and hospitals are in the plan’s network matters more than the company name.

What changed for 2027

  • The extra help is gone. The bigger premium tax credits from 2021–2025 expired December 31, 2025. The regular tax credits still exist for households earning between 100% and 400% of the poverty level — but they are smaller than the boosted ones.
  • The income cliff is back. Earn even $1 over 400% of the poverty level and you get no tax credit at all. For 2027 coverage that line is $63,840 for one person and $132,000 for a family of four.
  • Sticker prices are up. Insurers asked for a median increase of about 15% for 2027 (KFF’s review of 276 rate filings). Your own price is set by your plan, age, and county — you see it when you shop.
  • Do not just auto-renew. If you have a 2026 plan and do nothing, the Marketplace re-enrolls you — possibly at a much higher price. Check your renewal against the other 160 plans before December 15.

The dates that decide your coverage

  • Sunday, November 1, 2026: open enrollment opens. First day to enroll, renew, or change plans for 2027.
  • Tuesday, December 15, 2026: last day to enroll for coverage that starts January 1, 2027.
  • Friday, January 15, 2027: open enrollment ends. Plans picked December 16 to January 15 start February 1.
  • Your plan starts only after you pay the first premium — enrolling is step one, paying is step two.

How to compare 161 plans without losing your mind

  1. Start with your doctors and medicines. Cross off any plan that leaves out your clinic, hospital, or prescriptions. A cheap plan your doctor does not take is not cheap.
  2. Compare the whole cost, not just the monthly price — premium plus deductible plus the most you could pay in a bad year (the out-of-pocket maximum).
  3. If your income is between 100% and 250% of the poverty level, look at Silver plans. Silver plans carry extra savings that lower your deductible and copays — savings you only get on a Silver plan.
  4. Update your income estimate honestly. Your tax credit is based on it, and starting with tax year 2026, any extra credit you received must be paid back in full when you file.
