Free STD & HIV Testing in San Antonio (2026): Confidential Clinics & What to Expect

Getting tested for an STD or HIV can feel stressful. In San Antonio, it does not have to be expensive, and it does not have to be embarrassing. Several trusted local clinics offer testing that is free, confidential, and walk-in — no insurance needed at the free sites listed below.

Many sexually transmitted infections have no symptoms at all. That is why regular testing matters: it protects you, it protects partners, and most infections are treatable or manageable when they are found early. This guide walks through where to go in San Antonio, what each clinic tests for, how fast you get results, and what happens next if a test comes back positive.

Where testing is free in San Antonio

1. San Antonio AIDS Foundation (SAAF)

The San Antonio AIDS Foundation offers free testing for HIV, syphilis, gonorrhea, and chlamydia. All screening and confirmatory testing is provided free of charge, and counseling is available in English and Spanish.

  • Walk-in only — no appointment needed: Monday, Wednesday and Thursday 8:30 a.m.–4:00 p.m.; Tuesday 12:00 p.m.–8:00 p.m.; Friday 9:00 a.m.–1:00 p.m.; Saturday 11:00 a.m.–3:00 p.m.
  • How results work: HIV testing uses a finger-stick INSTI test with results in about 60 seconds. Syphilis rapid results are available in about 10 minutes. Gonorrhea and chlamydia results come back in 7–10 days through the state lab.
  • Privacy: All testing is strictly confidential. HIV testing may be done anonymously — your name and contact information are not required, although your date of birth must be provided.
  • Phone: 210-225-4715 — call to confirm the testing location and hours before you go, and press 1 if you are checking on results. Allow 7–10 business days for lab results and have the client ID from your results card ready.

If an initial HIV result is positive, SAAF provides confirmatory testing and Rapid Start linkage to medical care, social services, and pharmaceutical assistance, so you are not left to figure out the next step alone. Treatment for STIs is available at its Care Clinic based on appointment availability — ask your tester about options.

2. Kind Clinic — San Antonio (Isom Road)

Kind Clinic, a program of Texas Health Action, offers sexual health services in a safe, supportive environment, including PrEP, PEP, STI testing and treatment, and HIV testing and care.

  • Location: 722 Isom Road, Suite 104, San Antonio, TX 78216
  • Walk-in testing hours listed for San Antonio: Monday and Tuesday 10:00 a.m.–4:00 p.m.; Wednesday 12:00 p.m.–6:00 p.m. Walk-in availability may change based on how many patients the clinic can take each day, so arriving early or calling ahead is a good idea.
  • What drop-in testing covers: HIV, syphilis, hepatitis C, gonorrhea, and chlamydia. Other STI tests, and hepatitis A and B testing, are available by appointment.
  • Results: Results are posted to your patient portal within 3–5 business days. If a result is positive, Kind Clinic will call you to schedule a follow-up appointment for treatment.
  • Phone: 833-937-5463 (1-833-WE-R-KIND)

One important detail: drop-in testing does not include treatment during that same visit. If you have symptoms or know you were exposed and want testing and treatment in the same visit, book an “HIV & STI Testing” appointment instead of using drop-in hours.

3. BEAT AIDS

BEAT AIDS (Black Efforts Against the Threat of AIDS) provides free, confidential HIV and STI testing in San Antonio and welcomes people of every race and ethnicity. Its Ryan White and Prevention Testing Program is listed at 208 W. Euclid Ave., San Antonio, TX 78212, phone 210-212-2266. BEAT AIDS also shares testing events and additional locations on its site and social pages, so call before you go to confirm which location and hours are open that day.

4. Metro Health STI/HIV Clinic — the city clinic

The San Antonio Metropolitan Health District runs the city’s STI/HIV clinic at 512 E. Highland Blvd., Suite 150, San Antonio, TX 78210. The clinic provides testing, treatment, and counseling on a walk-in basis, although Metro Health says making an appointment is highly encouraged.

  • Hours listed on sa.gov: Monday–Thursday 8:00 a.m.–4:30 p.m.; Friday 8:00 a.m.–12:00 p.m.
  • Counseling and clinic phone: 210-207-2816
  • PrEP appointment scheduling: 210-207-7292. Metro Health’s PrEP team says that regardless of your financial situation, they will find a way to help — call if you have financial questions.
  • If Metro Health sent you a letter or called you: call 210-207-8830 right away, per the city’s note on its STI/HIV page.

Be clear about cost here: Metro Health’s mobile clinics offer free testing at events around the community, while fees at the main clinic can vary by service. Ask about cost when you call — do not assume, and do not let a possible fee stop you from calling, because the free options above and Metro Health’s mobile events exist for exactly that reason.

What to expect when you go

  1. Check in. At the free sites, you generally do not need insurance. At SAAF, HIV testing can be anonymous.
  2. A short, private conversation. Staff may ask a few questions so they test for the right infections. Counseling is available in English and Spanish at SAAF and Metro Health.
  3. The tests themselves. Depending on the clinic and your needs, testing can involve a finger-stick or blood sample, a urine sample, or a swab. It is usually quick.
  4. Your results. Some results are rapid — HIV in about a minute and syphilis in about 10 minutes at SAAF. Lab-based results take longer: about 3–5 business days at Kind Clinic and 7–10 days for gonorrhea and chlamydia at SAAF.
  5. If something is positive, you get a next step. That can mean treatment, a follow-up appointment, confirmatory testing, and linkage to ongoing care and support.

A note about the “window period”

SAAF explains it plainly: there is a window period between a possible exposure and the point when a test can detect an infection. If you test during that window, a result could come back negative even after an exposure. If you had a recent exposure, ask the clinic when you should test — and whether you should come back for a second test. Staff deal with this question every day.

What if I test positive?

A positive result is hard news, but it is not the end of the story, and you will not be handed a result with no help attached. At SAAF, a positive HIV screening leads to confirmatory testing and Rapid Start linkage to medical and social services and pharmaceutical assistance. At Kind Clinic, staff call you to schedule treatment follow-up. Metro Health provides diagnosis, treatment, and counseling, and can talk with you about PrEP for partners and prevention going forward.

Frequently asked questions

Is STD and HIV testing really free in San Antonio?

Yes at the nonprofit sites in this guide. SAAF states that all of its screening and confirmatory testing for HIV, syphilis, gonorrhea, and chlamydia is free of charge. BEAT AIDS offers free, confidential testing, and Kind Clinic offers drop-in HIV/STI testing — call 833-937-5463 to confirm current drop-in details. Metro Health’s mobile clinic events are free; fees at its main clinic can vary by service, so ask when you call 210-207-2816.

Do I need an appointment, insurance, or ID?

SAAF and Kind Clinic’s drop-in hours are walk-in, with no appointment needed. SAAF says HIV testing may be done anonymously and your name and contact information are not required, although your date of birth must be provided. Metro Health accepts walk-ins but says an appointment is highly encouraged — call 210-207-7292 for PrEP scheduling or 210-207-2816 for clinic questions.

How fast will I get my results?

It depends on the test. At SAAF, HIV results take about 60 seconds and rapid syphilis results about 10 minutes, while gonorrhea and chlamydia results take 7–10 days. At Kind Clinic, drop-in results are posted to the patient portal within 3–5 business days, and staff call you if a result is positive.

What should I do if I was recently exposed?

Go in and tell the clinic about the timing. Because of the window period, staff may test you now and ask you to retest later. If you have symptoms or a known exposure and want testing and treatment in the same visit at Kind Clinic, book an HIV & STI Testing appointment rather than using drop-in hours. For HIV prevention after or before exposure, ask any of these clinics about PEP and PrEP.
